New Patient Registration

Patients wishing to register at the practice are required to collect and complete two forms available at reception.

Form 1 is a registration form.

Form 2 is for you to provide any relevant medical history our general practitioners may need until your medical records are received from your previous general practitioner.

We require proof of your present address, e.g. a current utility bill at the time of registration, and if you are newly arrived in this country we will also require proof of identification, i.e. your passport or visa.

Once these forms are completed and returned, you will be asked to contact the surgery within 48 hours to check on your acceptance. If you are successful, you will need to arrange an appointment for a New Patient Check to be carried out with one of our practice nurses.

It is also important that we are provided with your current contact number.


You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Please note we do not accept postal registration forms, they have to be brought into the surgery together with a form of photo ID.
